MRI diagnosed a big L5 disc and an additional MRI diagnosed a lapped posterior herniated nucleus pulposus at L5-S1. Man had a lumbar discectomy and laminectomy surgery and was diagnosed as 10% permanent partial physical impairment of the whole body.

Award Reason :
